Calamvale Family Practice is committed to delivering safe, high-quality healthcare and protecting your privacy. These terms and conditions outline the responsibilities, expectations, and rights of all patients when using our services, including appointments, communication, billing, and digital services.

By attending our practice or using our website and services, you accept and agree to comply with these terms.

  1. Appointments & Attendance

We ask all patients to arrive on time for their appointments. If you are late, we may need to reschedule or offer a shorter consultation, depending on availability.

Cancellations & Non-Attendance

  • We request notice as soon as possible for appointment cancellations so appointments may be offered to other patients in need.
  • Missed appointments or late cancellations may incur a non-attendance fee.
  • Repeated missed appointments may result in limited access to future bookings.
  1. Fees & Billing

Calamvale Family Practice is a mixed billing clinic.
Bulk billing is available for:

  • Children under 16 (Monday to Saturday)
  • Concession Card Holders (Pension, Health Care Card, DVA) (Monday to Friday)

All other patients are required to pay privately on the day. Medicare rebates may be processed at the time of payment.

Telehealth billing: Standard billing policies apply to eligible telehealth appointments.

Payment methods accepted: Card & Cash. Please bring your EFTPOS cheque or savings account card with you to access your medicare rebate at time of payment.

  1. New Patients

New patients must download and print our  New Patient Form , complete it prior to their appointment, and bring it with them on the day. If you are unable to print the form, you may complete one at reception when you arrive.  

You are required to provide accurate and complete medical history to help us offer safe and appropriate care.

  1. Communication Policy

To ensure continuity of care, our practice may contact you via phone, SMS or email regarding appointments, results, recalls, reminders, or other relevant health information.

If you prefer not to receive reminders or digital communications, please advise our reception team.

  1. Privacy & Confidentiality

We comply with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). Your health information is stored securely and only accessed by authorised staff directly involved in your care.

We support MyMedicare and My Health Record . Ask our team if you’d like help registering or opting out.

  1. Access to Medical Records

Patients can request access to their medical records at any time.
Requests must be submitted in writing. A fee may apply to cover administrative costs.

Correction of inaccurate information is also your right, and we encourage patients to inform us of any changes to personal details or medical history.

  1. Zero Tolerance Policy

Calamvale Family Practice maintains a zero tolerance policy towards any form of aggressive, abusive, or disrespectful behaviour towards staff, doctors, or other patients.

Any such behaviour may result in refusal of service or removal from the patient register.

  1. Website Use & Content

All website content is for general informational purposes only and does not replace personal consultation with a qualified medical professional.

We make every effort to ensure the accuracy of the information presented, but we do not accept liability for any loss or damage arising from reliance on the content on this site.

  1. Third-Party Services & Links
